Service Agreement Program Form Instructions

Please use the following link to download the form. Instructions for completion are available below.

Instructions for completing the form:

  1. Contact Person: Please provide the name of the person who has firsthand knowledge of the printer. This individual normally works at the location where the printer is installed and will be the one to whom the TEC Services technician will report before and after servicing the printer.
  2. Phone #: Telephone number of the Contact Person.
  3. E-mail Address: The e-mail address of the Contact Person.
  4. Department: Telephone number of the Contact Person.
  5. Account(s) to Be Charged: The service fees can be charged to a single account number or divided among two to three account numbers.
  6. EQUIPMENT: Indicate the type of equipment – printer, notebook, desktop, server – that you desire to place under the Service Agreement.
  7. MAKE: Record the Manufacturer of the equipment.
  8. MODEL: Please provide the common model number of the equipment. For example, “HP 4000N” for an HP LaserJet ™ printer or “Latitude 266XT” for a Dell notebook would represent the level of information desired.
  9. SERIAL #: Record the serial number that is typically found on the rear panel of personal computers and HP LaserJet printers. On some HP LaserJet printer models, this plate is found under a service access door such as the one that is used for replacing the toner cartridge. If you can’t find the serial number, one of our service technicians will attempt to locate this information when he/she arrives at your site to perform the pre-Service Agreement Inspection.
  10. LOCATION: Record the Building and Room Number for UGA Department located on the Main Campus. If you are located off of the UGA Main Campus, please provide the street address. Printers located at remote UGA sites such as Griffin should provide the word “Griffin” in this field.
  11. PCs & NON-HP PRINTERS: If the equipment item that you are placing under the service agreement is a personal computer (of any type) or a printer manufactured by a company other than Hewlett-Packard, place an “X” in this space.
  12. HP PRINTERS (PM/PM PLUS): If you are placing an HP LaserJet printer under the service agreement, place an “X” in the PM or the PM PLUS box depending upon the level of coverage desired.

    If you are adding more than eight (8) units of equipment to the Service Agreement, you can hit the TAB key after making your last entry (in row 8) and the program will create a new row for another equipment item. Please use a second form if you have a large number of equipment items to place under the agreement.

  13. APPROVED BY (Typed or Printed Name): The name of the person who has fiscal authority to sign the Service Agreement Subscription Form.
  14. SIGNATURE AND DATE: The person approving the Service Agreement must sign and date a copy of the printed Service Agreement Subscription Form.
